Our Support

Our supported living services are designed for individuals who need assistance with daily living while maintaining independence. We work closely with service users, families, and local authorities to create safe, supportive environments.

We support with:

  • Daily living skills development

  • Budgeting and financial management

  • Community engagement and activities

  • Emotional and behavioural support

  • Maintaining tenancy and housing responsibilities

 

Our goal is to help each individual build confidence and independence while feeling safe and supported.

    • Conduct initial referral review and eligibility assessment

    • Arrange introductory meeting with the individual, family, and professionals

    • Complete comprehensive needs and risk assessments

    • Identify personal goals, preferences, and desired outcomes

    • Assess compatibility with supported living environment

    • Develop a person-centred transition plan

    • Identify required funding and confirm placement arrangements

    • Liaise with local authority, health professionals, and other stakeholders

    • Introduce the individual to the supported living setting.

    • Involve the individual in decision-making

    • Develop a detailed support and care plan

    • Plan staffing requirements and allocate key workers

    • Support the individual with life skills preparation (e.g., cooking, budgeting)

    • Create a transition timetable with clear milestones

    • Provide emotional support and reassurance throughout the process

    • Coordinate and support the moving-in day

    • Ensure a warm, structured, and welcoming environment

    • Introduce staff, housemates, and local area

    • Gradually implement support routines to avoid overwhelming the individual

    • Maintain familiar routines where possible

    • Monitor wellbeing closely during the initial adjustment period

    • Keep regular communication with family and professionals

    • Provide consistent, person-centred support tailored to evolving needs

    • Support integration into the community (activities, social inclusion)

    • Encourage independence and skill development

    • Monitor mental, emotional, and physical wellbeing

    • Hold regular review meetings with all stakeholders

    • Adjust care and support plans as required

    • Address any concerns promptly

    • Conduct scheduled care reviews

    • Track progress against personal goals and outcomes

    • Encourage feedback from the individual and their support network

    • Promote continuous skill-building and independence

    • Review risk assessments and update as needed

    • Plan for future progression (greater independence or step-down support)
